(Afternoon tea pics 2064.4: DME feat. Quad Wedge | Strata-G system | Kerr Simplishade Dark)
A challenging case with subgingivally-extending caries requiring DME. This was accomplished successfully using the Quad wedge as the bifid design ensures intimate cavosurface margin adaptation and minimal overhangs. The axial transition to the contact point region was unable to be achieved with the normal dead soft PTFE-coated bands due to the greater interdental space between these teeth. The Strata-G system was employed and an intentional overhang with cervical seal against the previously-completed DME resulted in a strong and acceptable contact area, the deficiency of which was the cause of recurrent caries on the root surface.
The restorative material used was the dark variant of the cloud-shaded Simplishade system from Kerr, which balances excellent sculptability, polishability and esthetic integration. The post-operative film confirms excellent marginal integrity of the restoration.
