It has been said that one of the hardest things to match in cosmetic dentistry is a single incisor in a sea of natural teeth or teeth restored with a different material. This case shows how far zirconia has come from the “once feared way too opaque result” scenario.
 
This nice 49 year old female despised her old zirconia crown with dark gingival show-through on tooth #8 (11), which was low in value and also featured marginal failure due to materia nero infiltration. Biologic width sounding: low crest situation with a firm, fibrotic biotype. Margin placement: 1mm subgingivally – a bit of a risk, but it paid off.
 
Material: A dark stump shade of ND4/ND5 was recorded here, and the restoration ingot chosen was an IPS e-max Zr CAD MTA1 (Ivoclar Vivadent) with full buccal layering with feldspathic porcelain (e-max Ceram). The entire crown excluding the facial was monolithic for strength, including the linguoincisal aspect.
