OCCLUSAL EFFECT CARIES – can occur when an unbonded large restoration allows the tooth to flex and places the mesial and distal marginal ridge under functional tensional stress eventually leading to a hairline fracture. This crack acts as a superhighway for bacterial transmission deeper into the tooth and voila.

This tooth exhibited a poorly sealed occlusal “capping” composite but underneath we found a Cavit base, which also did not contribute positive to the tensile resistance of the tooth under function.
